The MGH Chelsea Healthcare Center Adult Medicine Practice, a large academic internal medicine practice located at MGH Chelsea, serves a diverse population in the Chelsea Community.
Key Responsibilities
- Patient Care and Clinical Practice: Participate in daily huddles, share vital information, and incorporate patients as active team members. Provide independent, direct, and indirect nursing care, including physical exams, ordering diagnostic studies, evaluating results, and prescribing treatments within the scope of practice. Establish diagnoses, develop care plans, and manage both acute and chronic health issues. Refer patients to appropriate providers and write prescriptions in accordance with Massachusetts laws. Order, interpret, and communicate diagnostic results. Perform therapeutic measures such as wound dressings and sutures.
- Team Collaboration: Work as an integral team member, collaborating with primary care providers, social workers, case managers, and support staff. Assist in managing complex patients, participate in team huddles, and communicate regularly about patient care. Educate patients and their families, and coordinate care during transitions.
- Patient and Family Support: Assist patients and families in health management, provide education on health issues, and address urgent or complex problems in office or via phone. Provide emergency stabilization as needed and consult with physicians for diagnosis and treatment.
- Work Management and Quality Improvement: Follow up on labs and imaging, ensure smooth patient flow, respond to staff questions, monitor patient outcomes, and participate in quality improvement initiatives. Recommend practice changes when necessary.
- Professional Development: Maintain current knowledge and skills, participate in professional activities, and adapt to new practices. Develop protocols and standards collaboratively, and engage in research or publications as appropriate.
- Billing and Compliance: Ensure accurate billing in compliance with regulations.
Qualifications
- Current Registered Nurse licensure in Massachusetts as Nurse Practitioner
- Family/Adult/Pediatric Nurse Practitioner Certification
- Master's degree in nursing from an accredited institution
- Minimum 2 years nursing experience and 1 year as a Nurse Practitioner
- Preferred Bilingual: Spanish Speaking
- HIPAA compliant and BLS certified
