An exciting opportunity has arisen at The Denmead Practice.We are looking for an enthusiastic, committed Advanced Nurse Practitioner tojoin our experienced and dedicated team.

We are a well-established Practice situated in Denmead,Hampshire with easy access to the motorway and the South Downs National Park.The team has an excellent skill mix whereby the GPs and Nursing team workclosely together to provide first-rate patient access.

Sessions:16 hours over 2 days on Tuesday and Friday, plus sickness and holiday cover.

Salary:Negotiable dependent on experienceInformal visits to the Practice are welcome

Main duties of the job

The ideal candidate will have experience in General Practiceand will:work collaboratively with the whole practice team to deliver an excellentexperience that meets the needs of the patients.Provide care for the presenting patient; from initial history taking,triaging, clinical assessment, diagnosis, treatment and evaluation.Use critical thinking and practices within professional boundaries.Be proactive and a driver for change, assist in the training, development andclinical supervision of practice personnel and medical students

Assist with visiting Nursing Home patients andthe housebound when required.

Adhere to policies and procedures and provide guidance to the practice teamas required.

Job responsibilities

JobSummary / Purpose:

The Advanced Nurse Practitioner (ANP) is an integral member of our healthcareteam, the ANP will provide a range of advanced clinical assessments,diagnostics, and interventions, ensuring high-quality care and patient-centredservices working autonomously to support and work alongside the GeneralPractitioners.

As a keymember of the primary care team, the ANP will have a strong background ingeneral practice nursing and demonstrate exceptional clinical expertise,leadership, and communication skills.The role is diverse and will also include being involved in research,leadership and education as well as being actively involved in planning andimplementation of projects and areas of practice development.

KeyResponsibilities:

Conduct comprehensive patient assessments, including history-taking, physicalexaminations, and health screenings, to diagnose and manage acute and chronicmedical conditions.

Develop,implement, and evaluate individualised care plans in collaboration withpatients and other healthcare professionals.

Prescribe medications and manage pharmacological therapies according toestablished guidelines and regulations, while monitoring patient outcomes.

Deliver high-quality care across the lifespan, addressing the needs of diversepatient populations, including children, adults, and the elderly.

Provide health education and counselling to patients and their families,promoting self-management and preventive care strategies.

Collaborate with GPs, nurses, administrative staff, and external providers toensure seamless and coordinated patient care.

Participate in multi-disciplinary team meetings and case discussions to improvepatient outcomes and enhance service delivery.

Interpretand evaluate diagnostic test results, liaising with the patient/carer and theirGP initiating treatment accordingly.

Maintainaccurate and up-to-date clinical documentation in accordance with practicepolicies and legal requirements.

Staycurrent with clinical practices, guidelines, and regulations relevant toadvanced practice nursing.

Engagein quality improvement initiatives, contributing to the development of practiceprotocols and standards.

Engagein clinical supervision and self evaluation and use this to improve care andpractice

TheNP must comply at all times with the Practices Health and Safety policies, inparticular by following agreed safe working procedures and reporting incidentsusing the organisations Incident reporting System.

TheNP will comply with the Data Protection Act (1984) and the Access to HealthRecords Act (1990).

Assistand work with the Lead for practice Infection Prevention and Control toensure practice is compliant in its delivery of services, audits and regularchecks undertaken to evidence this. Education undertaken and cascaded wherenecessary to colleagues and external bodies. Liaise with both internal andexternal colleagues in the monitoring and maintaining of practice Infectioncontrol policies and procedures to minimise risk to staff, patients andvisitors to the practice.

SPECIAL WORKINGCONDITIONS

TheNP is required to travel to care homes and do some home visits

TheNP will have contact with body fluids ie, wound exudates; urine etc while in clinicalpractice.

Becauseof the nature of the work, this post is exempt from the provisions of Section4(2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995. Applicants for posts are not entitled towithhold information about convictions, which for other purposes are spentunder the provisions of the Act and in the event of employment any failure todisclose such convictions could result in disciplinary action or dismissal bythe Practice.
