As a Specialist Practitioner, your role will be to support GP practicesin Gateshead on a sessional basis during practice opening hours. You will be aRegistered Nurse, Paramedic or AHP with the skills to autonomously assess andtreat patients within General Practice. Telephone consultations and face toface contact. You will be required to work effectively within a multi-disciplinaryteam managing patients identified as at risk so that they receive appropriateinterventions to be able to improve their health and wellbeing and achievetheir maximum level of independence. These patients may have complex needs andpotentially numerous co-morbidities including frailty. This post is a clinicalrole where hands on and face to face consultations will be undertaken toclinically assess/diagnose/signpost/treat in a timely fashion working in amulti professional team.

You can choose shifts from a numberof practices who we represent in Gatesheads Primary Care Network via ouronline rota system. Shifts are on a sessional basis, during practice openinghours.

  • Non-Medical Prescriber with an understandingof the broad spectrum of general medicine
  • Educated to Degree/Masters Level.
  • Accredited Clinical Skills Course/orregistered Nurse / Paramedic /AHP Practitioner.
  • Accredited history taking course.
  • Enhanced IT skills databases, excel, word,outlook and internet.
  • Experience at band 5 or 6 (Clinical Role) orabove.
  • Experience of communicating delicate, complexand sensitive information.
  • Demonstrate a high level of interpersonal andcommunication skills to all stakeholders.

7. MAIN DUTIES AND RESPONSIBILITIES

  • See, treat, support treatment for a variety of conditions.
  • Management and appropriate interventions across a range of long-term conditions in line with best practice and NICE guidelines.
  • Implement individual care packages to patients working autonomously but with access to a GP.
  • Establish and maintain communication within the multi-disciplinary practice team.
  • Maintain written records to a high standard and within professional guidelines.
  • Display a positive and considerate approach to patients and their families / carers.
  • Support the team to provide high standards of patient care.
  • Provide clinical care within sphere of competence/specialty, knowing when to seek support or help as needed.

In the course of their work the hub member may have access to confidential information relating to the practice, its staff, stakeholders, and patients. All such information must be treated as highly confidential, and any breach will be subject to disciplinary action. In addition, the nature of this role may mean that the post holder also has access to information regarding local general practices and this should also be treated as confidential.

10. PERSONAL AND PEOPLE DEVELOPMENT

The hub member will participate in any training implemented by CBC Workforce Solutions as part of their contract and professional development.

11. WORKING CONDITIONS:

  • Nurse-led management of minor illness, minor ailments and injuries
  • Advanced clinical practice skills Management of patients with long-term conditions, complex needs and chronic diseases (diabetes review, cardiovascular disease coronary heart disease, hypertension, COPD, asthma)
  • Nurse-led triage
  • Nurse prescribing
  • Knowledge of long-term condition management
  • EMIS trained
  • Ability to work within own scope of practice and understanding when to refer to GPs
  • Flexibility of working days and hours including extended hours provision/able to travel to different locations at short notice
