Lead & Transform Complex Care in Devon! Full-Time/Part-Time Options Available. £50,000-£55,000 per annum.

Role & Responsibilities

• Conduct in-depth health assessments.
• Develop and implement bespoke care plans.
• Coordinate care with GPs, therapists, and social workers.
• Ensure the safety and effectiveness of care packages.
• Contribute to local business growth and commissioning initiatives.
• Empower patients and their families.
• Monitor outcomes and drive continuous improvement.

Requirements

• NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing.
• Proven experience in complex care or related nursing roles.
• Proactive, compassionate, highly organised, and a natural leader.
• Excellent communication and interpersonal skills.
• Strong analytical and problem-solving abilities.
• Ability to work independently and as part of a team.
• Commitment to providing high-quality patient care.
• Experience with care planning and coordination.
• Knowledge of relevant legislation and guidelines.
• Ability to manage time effectively and prioritize tasks.
