Main duties of the job

You will be responsible for the leadership, professional development and day to daycoordination of the nursing workforce, ensuring a high standard of clinical care across oursites. The role covers:

Clinical oversight of long-term condition management, infection control andimmunisation standards

Coaching, supervision and professional development of the nursing team

Ensuring compliance with CQC, IPC, safeguarding and other governanceframeworks

Leading and supporting clinical audits and improvement plans

Contributing to QOF achievement and delivering financially sustainable care

Working closely with the wider multidisciplinary team to ensure patient-centredservice delivery

Responding to the evolving needs of the organisation with flexibility and resilience

Job responsibilities

The successful candidate will be a registered nurse with proven leadership experience and astrong background in primary care. This role involves both clinical duties and teamleadership, supporting the delivery of patient-centred care and contributing to the ongoingdevelopment of our services.

Key Duties & Responsibilities

Leadership and Team Management:

  • Provide visible leadership across all five sites, promoting a culture of professionalism,learning and continuous improvement
  • Line manage the nursing team including Practice Nurses, HCAs, and other alliednursing roles, ensuring appropriate skill mix and service coverage
  • Lead regular team meetings and facilitate open communication across the nursingworkforce
  • Develop and deliver structured supervision, coaching and performance reviewprocesses
  • Ensure effective induction, ongoing support and competency assessment for all newand existing staff
  • Ensure all training, development and induction of Nursing Team members areundertaken in line with practice policies and procedures.

Clinical Oversight and Governance

Clinical Oversight and Governance:

  • Ensure high standards of care in line with NMC code, national guidance, localprotocols and CQC requirements
  • Oversee infection prevention and control, immunisation standards, and chronicdisease management delivery
  • Work with the Leadership Team and Clinical Leads to ensure compliance withsafeguarding, patient safety and clinical effectiveness frameworks
  • Maintain and audit documentation standards and clinical protocols

Strategic Planning and Service Delivery:

  • Contribute to the planning and delivery of sustainable nursing services aligned topatient need and practice priorities
  • Work in partnership with other clinicians and managers to identify opportunities forservice development, workforce expansion and pathway redesign. Support delivery ofQOF, Enhanced Services, and financial targets through effective team performanceand data-driven planning
  • Embed evidence-based practice and national best practice standards in long-termcondition care

Performance Monitoring and Activity Measurement:

  • Develop and maintain nursing activity and performance dashboards to monitorworkload, outcomes and quality metrics
  • Regularly review clinical throughput, appointment usage, and patient feedback todrive improvement
  • Identify under performance and implement supportive interventions where needed
  • Provide quarterly reporting to the leadership team on workforce metrics, includingcapacity, clinical output, and audit findings

Clinical Commitment

  • The Nurse Manager will maintain a reduced clinical caseload (typically one to two sessionsper week) to retain patient contact and clinical competence. This allows most of the workingweek to be allocated to management, supervision and strategic leadership responsibilities.

Person Specification
Knowledge / Skills
  • Strong knowledge of QOF, CQC compliance, safeguarding, infection control, and governance requirements.
  • Excellent interpersonal, coaching and communication skills.
  • Ability to work across multiple sites and manage competing priorities.
  • Strong IT skills including clinical systems (e.g. SystmOne, Accurux however full training will be provided).
Qualifications
  • Registered Nurse with an active NMC registration
  • At least one formal qualification in a long-term condition area (e.g. diabetes, asthma/COPD, cardiovascular disease).
Experience
  • Significant experience in a primary care setting, including autonomous management of long-term conditions.
  • Demonstrable leadership or management experience within a clinical team
  • Experience in staff performance management or workforce planning.
  • Evidence of leading clinical audits or quality improvement initiatives.
  • Familiarity with workforce and activity dashboards or reporting frameworks.
  • Experience in contributing to or delivering service redesign or pathway transformation
  • projects.
