Job summary

Torbay and South Devon NHS Foundation Trust is seeking a Band 5 Registered Nurse to join Harbour at Home, our Frailty Virtual Ward. You will deliver holistic nursing care from assessment to discharge, coordinating with a multidisciplinary team to support older adults living with frailty in their homes or community settings.

You will mentor junior staff, maintain a safe clinical environment, and uphold Trust values while delivering compassionate care.

Qualifications

• Registered Nurse with current UK registration (NMC).
• Experience communicating complex information to patients, carers and health and social care staff.
• Experience in palliative care or COPD/heart failure care preferred.
• CPD evidenced through ongoing professional development.

Responsibilities

• Plan, implement, and evaluate individualised patient care.
• Support, mentor, and guide junior colleagues and the wider MDT.
• Maintain a safe and supportive clinical environment for patients, carers, visitors and staff.
• Respect patients' wishes and values while providing care at home or in the community.
• Participate in MDT discussions and contribute to service improvements.
