We are seeking Registered Nurse to service our patients throughout the Piedmont area.

Nurse Case Managers will service homes and/or Assisted Living Facility patients in care coordination with the provider within their assigned service area(s).

This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

What Are We Looking For?

Your Health is currently looking for a Registered Nurse to join our growing primary care family.

Essential Functions
  • Admission
  • 匹ommunication, collaboration, and coordination of care
  • 匹ontinuous review and revision of CCM plan of care
  • 彦acilitate a telehealth visit with the nurse practitioner for coordination of care.
  • 匹oordinate with the patient痴 hospice interdisciplinary team and other individuals in the patient's care plan.
  • 姫articipate in the hospice IDG meeting
  • Assisted Living & Skilled Nursing Facilities
  • 紐ounding with physicians in assisted living or skilled nursing facilities
  • 匹oordinate with the patient痴 health care team, physicians, physical and occupational therapists, home health or hospice representatives and other individuals in the patient痴 care plan.
  • 柊ssess and chart observations of the patient痴 condition.
  • 匹ollaborate with nurse practitioners to satisfy each patient痴 quality measures.
  • 彦acilitate telehealth or tele-pharmacy visits
  • 髭nsure follow-up visits are scheduled
  • 姫articipate in quality improvement projects i.e. root cause analysis of adverse incidents
  • 筆aintain current list of providers and specialists updated in the EMR
  • 匹ommunicate with patients, families, and caregivers.
  • 俵tilize the company痴 software systems and update information as required.
  • 姫articipate in coaching calls.
  • 姫erform other duties as requested or required, in the sole discretion of SC House Calls.
QUALIFICATIONS

Must be a licensed nurse. License must be in good standing with the appropriate board.

Minimum of three (3) years clinical experience preferred.

Demonstrated ability to supervise and direct professional and administrative personnel.

Ability to read and communicate effectively.

Ability to manage and demonstrate effective leadership skills.

Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
