Glossop Primary Care Network (PCN) are looking to recruit a full-time band 5 Nurse with experience in primary care to join our team in providing a high-quality service to our patients.
The Band 5 Nurse working within the Primary Care Network (PCN) Home Visit Team is responsible for delivering high-quality, evidence-based nursing care to patients in their own homes. The role focuses on supporting patients with acute, long-term, and complex health needs, working collaboratively with multidisciplinary teams to promote independence, prevent hospital admissions, and facilitate safe discharges.
Glossop PCN consists of 4 practices in Glossop and Hadfield who have a very cohesive working relationship. We cover a population size of 33,500 patients. Our PCN values are to work collaboratively, nurture learning, and implement innovative ways of working and to deliver care with compassion. We would want the person joining our team to be able to identify with these values.
This post supports the PCN's commitment to caring for patients close to their homes, reducing hospital admissions, improving patient outcomes, and enhancing the overall patient experience within the community setting.
The role will consist of a community delivery of healthcare through acute home visits, and engaging proactively with population health as part of the Team Up initiative.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
