Role Summary

As a Nurse Associate, you will be responsible for working clinically to deliver care to patients in their own homes and in a clinic environment. You will provide holistic care, advice, and information to patients and carers within the home environment or other appropriate care settings.

We work closely as a wider team to prevent hospital admission where possible and facilitate hospital discharge for patients to be cared for within their own home environment. We aim to support high-quality patient care at home or the most appropriate environment, enabling independence and quality of life.

Experience in community care is desirable but not essential, as full training will be provided and support will be available to develop in the role.

You will need a full driving license and access to a car with business insurance to undertake this role. Lone working is expected after an induction period, with ongoing support from experienced nursing staff.

Working hours per week: 7 days per week, flexible hours.
