Nurse Associate (Community)

As a Nurse Associate you will be responsible for working clinically to deliver care to patients in their own homes and in a clinic environment. You will be expected to provide holistic care, advice and provide information to patients and carers within the home environment or other appropriate care settings.

We work closely as a wider team to prevent hospital admission where able to to facilitate hospital discharge for patients to be cared for within their own home environment. We strive to support high quality patient care in their home or the most appropriate environment enabling them to maximise their independence and optimise their quality of life.

Experience in community care is desirable for these posts but not essential as full training will be provided and the post holder will be supported to develop in the role.

You will need to have a full driving licence and have access to car, with business insurance to be able to fully undertake this role. Lone working is expected within this role after a period of induction with ongoing support from our experienced nursing staff.

Working Pattern: 37.5 hours per week. 08:30 to 16:30. 7 days per week
