Nurse Associate - Care of the Elderly Ward 5B

This Nurse Associate (NA) role is within the Frailty service, working alongside Registered Nurses, Geriatricians, and the multi-professional team to provide and monitor high-quality care, contributing to ongoing assessments in line with service pathways. The NA will continually monitor the health needs of patients in collaboration with the team.

Due to the success of our Frailty Service, we are expanding and seeking an enthusiastic Nurse Associate to join our dedicated team of nursing and medical staff.

The post involves providing direct patient care in the community for the Virtual Frailty Ward, Community Frailty, and Urgent Community Response services, including home visits. It also includes work in the hospital on the Ambulatory unit, ward, and in the Emergency Department alongside Advanced Clinical Practitioners.

You will act as an autonomous practitioner, delivering holistic, patient-centered care to promote independence and well-being for those living with frailty.

Main duties of the job

Job responsibilities

CLINICAL & PROFESSIONAL RESPONSIBILITIES

Provide high-standard nursing care under indirect supervision, supporting the assessment, planning, and evaluation of patient care, and promoting independence and health.

Support individuals with personalized care, ensuring adjustments for protected characteristics, and manage patient conditions following medication administration.

Perform clinical observations (blood pressure, temperature, respirations, pulse) and undertake clinical tasks (cannulation, venepuncture, ECGs) as competent.

Record nutritional and fluid intake, maintain patient dignity and safety, and act according to policies and organizational values.

Participate in team meetings, recognize changing priorities, and report patient conditions to registered nurses.

Identify safeguarding issues, contribute to risk management, and support complex care delivery.

Maintain understanding of specific conditions like dementia and mental health, and develop coaching skills.

Support clinical standards, understand referral systems, and engage in reflective practice.

Ensure informed consent, understand the Mental Capacity Act, and communicate effectively with patients and colleagues.

Maintain accurate records, manage electronic systems securely, and work independently within your scope, exercising judgment based on patient assessment.

ADMINISTRATIVE RESPONSIBILITIES

Maintain professional records, follow information governance policies, and uphold confidentiality.

TEACHING & TRAINING RESPONSIBILITIES

Complete mandatory competencies, participate in area-specific assessments, and support the training of learners and new staff.
