Job description

The CommunityNursing Service within North East Lincolnshire provide nursing care and supportto patients, families and carers in a variety of community settings supportingpatients to remain in their own homes, preventing hospital admission andfacilitation of early hospital discharges and managing end of life care.

The CommunityNursing Service works closely and in partnership with Primary Care Networks,Secondary Care, Social Care, Specialist Nurses and other healthcare staff toprovide a quality service personalised to meet individual needs. The service supports and assists patients tomanage ill health and long term conditions including elements of disability,thus allowing them to have the best possible quality of life, while maintainingdignity and respect.

Main duties of the job

Job title: Nurse Associate

Team: Community Nursing Panacea

Location: Cleethorpes Primary Care Centre

30 hours a week, Permanent

You will work under the direction of the shift/team leader/manager insupporting other Health/Social Care Professionals and the delivery of Healthand Social Care interventions to Service Users.

You willbe expected to independently manage and self-manage a defined caseload of lesscomplex cases, evidencing problem solving and reasoning skills in response tounpredictable patterns of work. You will complete assessments and deliver support for the functional/clinical needs of patients in the communityincluding chronic and palliative conditions.

You will beresponsible for developing and implement care/support plans under direction ofthe appropriate professional which may include clinical, social care and/orother practical activities with minimal guidance. You will monitor and report any unexpectedchanges in condition of the individual to the appropriate professional in atimely manner. You will manage care of individuals and carers where thesupport/care plan has been put in place and may need close monitoring, frequent review or where interventions to minimise and manage risk may berequired.

You will be requiredto utiliseclinical skills including venepuncture and wound management with an on-goingresponsibility to maintain and improve clinical skills and knowledge as serviceneeds demand.

Qualifications
  • Registered Nursing Associate on the NMC register
  • Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
  • Care Certificate or willing to achieve within 6 months of commencement in post

  • To have a range of clinical skills, such as, wound care, eye care, pressure area care, venepuncture, bowel/bladder care, clinical observations including blood pressure and blood glucose monitoring
