Job description

The Barnsley Primary Care Network is seeking to recruit Qualified Nurse Associates to join our clinical team. These roles will be based at assigned practices within our network, supported by a Practice Nurse for mentorship. Practice location to be confirmed.

The Nursing Associate will work alongside Registered Nurses and the multi-professional team, providing and monitoring care in line with service pathways. They will assess and support patients' health needs, promoting healthy lifestyles and ensuring patients lead fulfilling lives.

This position is based at Huddersfield Road Surgery, full-time, but part-time (Tuesday to Friday) applications are also welcome.

Main duties of the job

The Nurse Associate will support the delivery of high-quality nursing care, working independently and managing their workload. Excellent communication skills, target achievement, and collaboration with patients and staff are essential.

The PCN encourages individuals interested in primary care to develop their skills within this setting.

Job responsibilities

Primary duties include:

  • Administering immunisations and health promotion advice
  • Supporting infection control and flu planning
  • Supporting General Practice Nurses with QOF
  • Conducting health recalls and screenings
  • Monitoring and assessing patient care, recognizing when to refer to senior staff
  • Recording baseline observations and identifying anomalies
  • Planning and discussing care under supervision, escalating concerns as needed
  • Managing physical and mental health issues, including chronic disease management
  • Supporting risk assessments and safety protocols
  • Communicating effectively with patients, families, and the MDT
  • Supporting team supervision and contributing to quality improvement and safeguarding
  • Participating in integrated care pathways and using clinical systems like SystmOne
