Job description

We are looking to recruit an enthusiastic Nurse Assessor within the Cornwall Onward Care Team, based in Derriford Hospital. This person requires previous experience in patient flow. An exciting opportunity to support patient flow from an acute hospital working alongside an experienced team. The aim is to achieve a collaborative approach to hospital discharge and ensure that patients only stay in hospital for the time required to complete their acute treatment, and that, where necessary, they experience timely and seamless transfer of care to another setting. The role requires assessment skills to identify the needs of the patient in a holistic manner and support setting realistic goals for discharge, within the MDT.

Key Responsibilities:
  • To ensure patient's needs are assessed in a timely and effective way.
  • To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards their onward care.
  • Develop working relationships with ward teams to enable them to support flow across the acute trust.
  • In collaboration with clinical teams, health and social care agencies, the Integrated Transfer of Care Hub in planning and managing the flow through the Acute trusts.
  • Contribute to follow-up assessments, care planning, identification of resources, and service delivery in order to facilitate the appropriate and timely discharge of patients.
  • Ability to work flexibly and proactively in response to the needs of the service, working across organisational boundaries (RCHT, CFT, Adult Social Care, and Voluntary Sector).
  • To use highly developed communication skills effectively in complex and potentially challenging situations.
  • Be responsible for guidance, teaching, and support to other staff as appropriate.
  • To act as an advocate to patients and families/carers.
  • Takes active responsibility in the resolution of minor or potential conflict, liaising with senior staff as appropriate.
