A healthcare provider in the United Kingdom is looking for a Childrens and Young Peoples Continuing Care Nurse Assessor. This role involves collaborating with multi-agency professionals to provide equitable and timely care, offering Personal Health Budgets, and supporting families. Ideal candidates should be Public Health or Paediatric Nurses with experience working with children and young people. The position includes travel within Gloucestershire and a comprehensive induction programme.
You will work collaboratively with multi‑agency professionals, including Local Authority, Education, parents, families, and advocates, ensuring that care is equitable, holistic and timely. This role will involve offering Personal Health Budgets including Direct Payments, working with existing care providers and supporting in developing a Commissioned care provider service network.
You will be supported to develop expertise in the National Framework for Children and Young Peoples Continuing Care (CYPCC), helping children and families access equitable care and support to meet their complex needs.
The role will involve travel within Gloucestershire and on occasion out of County for face‑to‑face assessments.
