The main duties of the posts are to assess and review individuals with complex health care needs within the community and hospitals.
A vacancy has arisen with the Complex Care Team for an experienced Registered Nurse (RGN, RMN or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Ty Gwent, Cwmbran.
The role will require the post-holder to undertake in-depth expert / specialist nursing assessments in hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual's eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding. Consequently, the role will require the post-holder to facilitate the Multi-disciplinary Team assessment process, and to provide input into the discharge process.
The post-holders will promote collaborative and inter-agency working across professional and organisational boundaries by providing professional advice, support and guidance to both professional and non-professional colleagues, patients, and their relatives / representatives.
The ability to speak Welsh is desirable, but not essential for this post.

The successful applicants should have:
Recent NHS or Care Home experience is essential.
A proven track record of being reliable and dependable.
A flexible approach to work would be a great advantage.
