Job Overview

The main duties of the posts involve assessing and reviewing individuals with complex healthcare needs within the community and hospitals.

Main duties of the job

A vacancy has arisen with the Complex Care Team for an experienced Registered Nurse (RGN, RMN, or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Ty Gwent, Cwmbran.

The role requires conducting in-depth expert/specialist nursing assessments across hospital sites in Gwent to determine eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess, and NHS Funded Nursing Care (FNC). The post-holder will facilitate multidisciplinary assessments and contribute to the discharge process.

The post-holder will promote collaborative working across agencies by providing professional advice, support, and guidance to colleagues, patients, and their families or representatives.

While speaking Welsh is desirable, it is not essential for this role.

Qualifications

Essential criteria:

  • Degree-level education
  • Adult Safeguarding Level 1 & 2

Desirable criteria:

  • Postgraduate certificate or degree in community care/nursing or relevant area
  • Additional courses or postgraduate certificates in managing challenging behaviour, dementia
Experience

Essential criteria:

  • Experience in undertaking needs-based assessments and care planning
  • Experience with capacity assessments, gaining consent, and applying DoLS and the Mental Health Act

Desirable criteria:

  • Community nursing experience
  • Knowledge of Mental Health and Mental Capacity Acts
Knowledge

Essential criteria:

  • Handling complex cases
  • Report writing skills
  • Knowledge of Continuing Care legislation and policy
