Role of the NRC Case Manager

The NRC case manager plays a vital role in the patient journey and interdisciplinary team. Responsibilities include coordinating admissions, managing the patient journey and rehabilitation programmes, and overseeing discharge processes. The role also involves maintaining key liaison roles with families and carers. The ideal candidate will have experience in senior healthcare leadership, coordinating patient pathways, aligning professional and patient expectations, and facilitating timely clinical decision-making. The post holder will promote a clinically excellent model of rehabilitation at the NRC, ensuring strong and visible clinical leadership throughout the patient journey.
