Modality Partnership Lewisham Division has a part-time (20 hours) vacancy for a Level 3 Health Care Assistant to be based at Bellingham Green Surgery. The postholder will be a high performing HCA, working across Modality Lewisham Division including one a week domiciliary visits. We are seeking dedicated postholders who will work collaboratively with the general practice team to meet the needs of our patients through the delivery of safe patient care. You will also hold a full driving license.

The post holder will support as part of the wider nursing team to promote the health and wellbeing of our patients by assessing their needs, providing pre-screening activities and health care for chronic diseases, health education advice and support, implementing requirements in line with patient requirements, community, or other settings, maintaining clinically accurate records during clinics.

This job is suitable for individuals who can demonstrate safe and effective patient interactions ensuring the delivery of high-quality patient care whilst working alongside the wider nursing team and practice team.

Main duties of the job

This is a hands-on patient facing role; we are seeking individuals to join our team who are caring, compassionate and confident, who demonstrate effective communication, excellent customer service skills, good IT skills and the ability to support patients with interventions and/or queries. The postholder will be a key member of the practice team, responsible for undertaking patient health checks inc. BP / ECG monitoring / height and weight, taking blood samples, health and wellbeing advice and promotion.
