About the Job:
Key Responsibilities:
- Observe, report, and record changes in clients' physical, mental health, and behavior.
- Provide care for service users as planned in multidisciplinary team (MDT) meetings.
- Be aware of and adhere to the principles of the Mental Health Act (MHA).
- Communicate clearly via telephone or personal contact.
- Provide physical care to help clients with daily activities such as personal hygiene, diet, clothing, laundry, environment, elimination, and maintaining rest, sleep, activity, and physical observations.
- Respect clients' customs, individuality, values, sexuality, and spiritual beliefs, supporting them to fulfill these.
- Provide social activities, emotional support, and stability to ensure effective engagement while maintaining professional boundaries.
- Contribute to assessment, planning, implementation, and evaluation of care.
- Maintain professional and supportive relationships with staff, professionals, and agencies to promote recovery.
- Escort duties under the direction of the nurse in charge.
- Perform observations as per individual care plans and client needs, in line with HPT Trust Policy.
- Participate in co-facilitation of therapeutic and social engagement group work, where required.
- Undertake prescribed physical observations (blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight, and height) and report results to the nurse in charge.
