About the Job:

Key Responsibilities:

  1. Observe, report, and record changes in clients' physical, mental health, and behavior.
  2. Provide care for service users as planned in multidisciplinary team (MDT) meetings.
  3. Be aware of and adhere to the principles of the Mental Health Act (MHA).
  4. Communicate clearly via telephone or personal contact.
  5. Provide physical care to help clients with daily activities such as personal hygiene, diet, clothing, laundry, environment, elimination, and maintaining rest, sleep, activity, and physical observations.
  6. Respect clients' customs, individuality, values, sexuality, and spiritual beliefs, supporting them to fulfill these.
  7. Provide social activities, emotional support, and stability to ensure effective engagement while maintaining professional boundaries.
  8. Contribute to assessment, planning, implementation, and evaluation of care.
  9. Maintain professional and supportive relationships with staff, professionals, and agencies to promote recovery.
  10. Escort duties under the direction of the nurse in charge.
  11. Perform observations as per individual care plans and client needs, in line with HPT Trust Policy.
  12. Participate in co-facilitation of therapeutic and social engagement group work, where required.
  13. Undertake prescribed physical observations (blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight, and height) and report results to the nurse in charge.
