Responsibilities

• Provide support and encouragement by following the person-centred support plan.
• Maintain accurate records in line with service requirements.
• May administer personal care and medication.
• Wall Street is a residential home in Hereford city centre.
• Support people with cerebral palsy, MS and acquired brain injury or stroke with person-centred planning.
