Job DescriptionJob Responsibilities- Fully engage in the Assessed and Supported Year in Employment (ASYE) Programme for Newly Qualified Social Workers by planning and reviewing progress and development at regular intervals, fully co-operating with the ASYE Assessor and the Supervisor, attending support groups and other activities for NQSWs, and preparing for own supervision and appraisal through self-evaluation, ensuring that continual professional development is evidenced.
- Undertake person-centred social work/community care assessment with customers and plan for the provision of support and/or services to assist them in meeting their outcomes and live as independently as possible.
- Develop creative approaches to meeting needs and maintain up-to-date knowledge of local provisions.
- Be responsible for accurate and timely recording of all information/assessment activity onto the Social Care records database.
- Apply assessment and management of risks for individuals and act in accordance with safeguarding policies and procedures.
- Gather sufficient information about customers to accurately assess their needs against eligibility criteria.
- Participate in Specialist Response rota enabling access to services.
- Ensure that Council corporate and service policies and procedures are followed at all times.
- Conduct Support Plan Reviews in a timely and person-centred way.
- Contribute proactively to the effective working of the team.
- Be an ambassador for the Council and the department at all times.
Person SpecificationQualifications Essential- Qualified Social Worker, taking up post within one year of achieving a recognised social work qualification.
- Social Work England registered.
- Up-to-date knowledge and understanding of relevant legislation and guidance in relation to vulnerable adults.
- Well-developed interpersonal skills and ability to effectively communicate.
- Ability to utilise Microsoft and Electronic Social Care record software.
- Must be able to write clear, complex, and accurate reports.
- Evidence of appropriate work experience and reflective learning from placement experience.
- Ability to identify problems and demonstrate a creative approach.
- Ability to develop and maintain effective professional relationships.
- Fully fluent in English.
Experience Essential- Some relevant professional experience post qualification in a similar work environment.
- Expert knowledge of relevant policy, systems, work practices, and legislation.
- Excellent ICT skills including use of Microsoft applications.
- Experience of multi-disciplinary and partnership working.
- Planning and workload management skills.
- Proven assessment and investigation skills.
- Proven ability to work with challenging client groups.
- Excellent interpersonal and communication skills.
- Accurate record keeping and report writing.
- Knowledge of financial assessment processes.
