A healthcare provider in the UK is seeking a New to Primary Care Nurse to join their team. This role focuses on coordinating a wound care service, delivering high-quality patient care, and participating in quality improvement initiatives. The ideal candidate will be an experienced Registered Nurse with a specialty in wound care, committed to collaborative care within the Primary Care Network. The position offers access to the NHS Pension Scheme and other benefits.
The postholder will work as New to Primary Care Nurse within the Primary Care Network (PCN) services ensuring an efficient and effective service for patients. The post holder will be part of a multi-disciplinary team within the PCN, delivering high-quality care and supporting the PCN in meeting objectives set out in the 2025/26 Network Contract Directed Enhanced Service (DES).
This role is suitable for an experienced Registered Nurse, with a speciality in wound care, entering general practice from secondary care or community.
The post holder will be responsible for the co-delivery and coordination of a PCN-wide wound care service, ensuring consistent, evidence-based care across the network and reducing unwarranted variation in practice. Alongside this, there may be opportunity to undertake additional nursing duties such as chronic disease management, health promotion and vaccinations.
