A local health service provider is seeking a General Practice Nurse to join their team. The role involves conducting immunisations, managing chronic diseases, and developing care plans for patients. Responsibilities Include:
West Moors Village Surgery is a small but friendly surgery in the heart of the village. We currently have 5500 patients, 6 GPs, HCA, PN full admin and reception staff.
We are part of Crane Valley PCN and Castleman Federation.
The post holder will be responsible for implementing processes for the effective management of patients with long-term conditions, including care for elderly and housebound patients, using evidence-based practice.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the patient population.
The Practice Nurse will oversee clinical areas such as infection prevention and control, health promotion, chronic disease management, well woman and well man clinics, and support the practice management team in reviewing and delivering clinical policies and procedures. West Moors Village Surgery will ensure the post holder has access to appropriate clinical supervision and support.
The core responsibilities include developing care management programs for frail and housebound patients, assessing patient needs, implementing health promotion programs, managing chronic disease treatment plans, supporting at-risk patients, providing routine nursing care, diagnosing and managing conditions, prescribing medications within scope, reviewing medications, guiding patients on medication use, coordinating travel medicine services, requesting and processing pathology results, overseeing chronic disease clinics, maintaining accurate records, ensuring effective use of read codes and chronic disease registers, participating in well woman and man clinics, ensuring compliance with policies, assisting with minor surgeries, prioritizing health issues, supporting clinical emergencies, recognizing mental health needs, implementing vaccination programs, supporting medication adherence, liaising with external services, delegating responsibilities, supporting safeguarding, understanding substance abuse policies, delivering health promotion, developing audit protocols, reviewing and implementing nursing protocols, participating in initiatives, supporting shared learning, reviewing practices, and participating in incident reviews.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A DBS check will be required.
Depending on experience, the salary is equivalent to Band 5.
