Job summary

HCRG Care Group is seeking a Neurology Physiotherapist to support patients with neurological conditions across community and outpatient settings in Wiltshire. The role involves delivering detailed assessments and bespoke rehabilitation plans in patients’ homes and clinics, contributing to positive outcomes and independence.

Requirements include HCPC registration, a physiotherapy degree, and experience in neuro rehab.

Qualifications

• HCPC registration is essential.
• Experience in neurological physiotherapy is required.
• Active participation in CPD and evidence-based care.

Responsibilities

• Deliver specialist physiotherapy assessments, treatment plans and rehabilitation programmes for patients with neurological conditions in community and outpatient settings.
• Manage a varied caseload independently with accurate documentation and outcome evaluation.
• Collaborate with a multidisciplinary team to plan care and reviews.
• Promote patient independence and involve carers in decisions about care and goals.
• Support admission avoidance and timely discharge from hospital when appropriate.
• Manage clinical risks and adapt interventions as needed.
• Maintain high clinical standards and act as a role model for best practice.
• Use therapy equipment safely and contribute to service quality improvements.
