We are looking for a suitably experienced person to join the Neurodevelopment Pathway Team to support, assess and manage the children and young people undergoing assessment in the Wigan Borough. This is a permanent full time post (37.5 hours) working across Monday to Friday.
The post holder will have extensive NHS/social care experience working with children and young people and their families with additional and SEND needs who have been referred for assessment of autism.
You must be knowledgeable is all aspects of SEND and the health conditions often associated with additional needs including autism such as sleep, behaviour, and sensory needs.
You must be able to support families on a 1:1 basis or in groups to deliver pre and post-diagnostic support sessions. As you may be based in different clinics, you must have access to your own car and have appropriate business insurance.

The post holder will demonstrate responsibility for:
Providing an efficient and co-ordinated service to the children, young people and their families accessing the School Age Neurodevelopment Pathway
Maintaining confidentiality when dealing with patient identifiable data
Effective communication between health services and outside agencies
Co-ordinating and management of the Neurodevelopment caseload
Signposting parents, carers and families to specialist support and services
Point of contact for parents/carers, General Practitioners, education staff and other health professionals, working as part of a team to develop and maintain good working relationships
Providing appropriate evidence based pre and post diagnostic advice and coaching support to families in a variety of settings on a range of topics associated wit autism and special educational needs (SEND)
Seeking guidance and support from the Neurodevelopment Pathway Manager and/or Team Leader
Reporting to the Neurodevelopment Pathway Manager any concerns about a child or young person including safeguarding or other environmental issues
Having knowledge of and adhering to local safeguarding policies and procedures to enable relevant referrals to be made including to childrens social care
Sharing knowledge and experience around specialist support an community based services available to children, young people and their families with autism, or SEND
Key results from the job holder
Planning and Organisational Duties
Receive and assess referrals and liaise with Neurodevelopment Pathway Manager as appropriate.
Participate in performance reviews and the formation of personal and team development plans.
Participate in the development and review of the services Care group and organisations policies procedures and guidelines
To be responsible for managing own daily workload and planning, prioritising, and delivering the service effectively, whilst identifying any issues and liaising with the Neurodevelopment Pathway Manager or Team Leader
Communications and Key Working Relationships
The post holder will participate in and influence care delivery by:
Actively managing the progress of children and young people referred to the Neurodevelopment Team
Providing up to date information on progress within the pathway including the assessment process
Ensuring all relevant information is gathered in preparation for diagnostic panels
Monitoring and managing referrals in to the team ensuring parents/carers ad families are updated within appropriate timeframes
Ensuring assessments are completed at appropriate pints of the pathway where required
Working in partnership with the Community Paediatric Team to provide case management for children, young people and their families
Assessment and triage of sometimes complicated information recognising where escalation is required and seeking support and assistance from the appropriate place
Using effective and adaptive communication skills acknowledging diversity, ethnic and cultural differences and the needs of the individuals and their families whilst maintaining confidentiality and respect
Responsibility for Finance
To be responsible for the most effective and efficient use of resources and equipment and to rapidly identify any replacement needs and resource issues
Responsibility for Human Resources
Participate in the continuing development of induction, orientation and preceptorship programmes for all permanent team members
Responsibility for Health & Safety
Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
Responsibility for Teaching
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
