Job description

Join to apply for the Neuro Navigator (City and Hackney) role at Homerton Healthcare NHS Foundation Trust.

Neuro navigation is a well-established role in City and Hackney supporting patients requiring specialist and/or complex neuro rehabilitation pathways. This includes providing advice and signposting within specialist rehabilitation, supporting discharge, and long-term complex case management in collaboration with continuing healthcare. The role also involves offering expert clinical advice to the adult community rehabilitation team and neuro rehab services at Homerton.

The Neuro Navigator is expected to understand complex rehabilitation pathways for clients with neurological diagnoses and long-term care needs, including those with deteriorating neurological conditions and single neurological events.

You will provide expert clinical opinions, monitor referrals and admissions for inpatient rehabilitation levels 1, 2a, 2b, community, and out-of-borough placements. The role involves working closely with other healthcare and social care providers, continuing healthcare, and promoting an integrated approach to managing neurological conditions.
