Job overview

We are seeking committed and diligent staff to work as Network Navigators in our three Acute Mental Health Wards at Blossom Court, St Ann's Hospital. The roles are full time and in person with face-to-face working as a minimum expectation. These are new roles to work alongside our Patient Flow service and will be part of a wider system supporting and managing patient access and flow within the North London Mental Health Partnership.

Main duties of the job

Reporting directly to the Discharge Coordinator, you will be assigned to an acute ward and will operate within the ward daily, ensuring 72-hour formulation meetings are in place, liaising with ward staff, and ensuring all actions are completed to facilitate the safe discharge of in-patients when clinically ready.

Hours are Monday to Friday, 9am to 5pm. Please note professional registration is not required for these roles.

You may be required to work at different locations under the Trusts to meet service needs. The Trust reserves the right to require staff to work at any locations under Barnet, Enfield, Haringey, or Camden and Islington Trusts, temporarily or permanently.

Detailed job description and main responsibilities

Network Navigators will ensure that each patient’s network of care is involved early in admission to inform care planning and discharge. They will organize this system around the patient to join key meetings during admission.

Their interactions will be therapeutic, strengths-based, and recovery-oriented. The network will include relevant individuals and services supporting recovery, such as carers, community mental health and substance misuse teams, voluntary organizations, criminal justice staff, and housing providers where relevant.

They will work closely with Discharge Coordinators and ward teams to identify barriers and strengths early on, work towards timely discharge, and facilitate carer involvement.

Person specification
Experience and Knowledge
  • Significant experience caring for people with serious mental illness.
  • Understanding of the needs and difficulties faced by people with serious mental illness and their carers.
  • Experience working in an acute mental health inpatient setting.
Qualifications/Registrations
  • Diploma in Mental Health Practice or NVQ Level 3 or 4 in Mental Health, or extensive current experience working with patients with serious mental illness.
  • GCSEs or equivalent, including Maths and English (minimum 5 A–E qualifications).
Skills/Abilities
  • Digital competence in Outlook, Word, data entry in EPR systems, Excel, and internet research.
