A health service provider in Teignbridge is seeking two part-time Registered Nurses to join the Community Service team. The roles involve conducting clinical assessments for older persons and coordinating their care, allowing them to maintain independence. Candidates should be NZ Registered Nurses with a current APC. Training in InterRAI Assessment tool will be provided if needed. Applications are open until August 8th.

Based in Ashburton hospital,Ashburton Community services is run from hospital and specialist services as part of Ashburton Hospital.Ashburton & Rural Health Services provides a range ofInpatient, Day patient, Outpatient and Community based health services to thecommunity.
The Community Service team atAshburton Hospital has an opportunity for a Registered Nurse/Allied HealthProfessional to join their team as a Needs Assessor and Service Coordinatorfor Older Person’s health.
This role requires acomprehensive clinical assessment of older person’s medical, rehabilitation andsupport needs to enable them to remain in their current environment. You willbe supporting older people to maintain their independence using a restorativemodel at home by providing necessary support or assist them in the decisionmaking process around placement. You will liaise with the multi-disciplinaryteam, coordinating referrals and empowering the client and their family toensure a positive outcome.
This role will require you to usethe electronic InterRAI Assessment tool.
InterRAI trained is preferred butnot essential, as training can be provided.
Skills and experience
A NZ Registered or Enrolled Nurse with current Annual Practising Certificate.
