We are seeking an enthusiastic individual to join the Adult Community Rehabilitation Team (ACRT) as the MS Specialist Practitioner for City and Hackney. The ideal candidate will have experience working with MS patients, preferably in both acute and community settings. This is a full-time, permanent position.
By adopting a holistic, collaborative, and coordinated approach, you will aim to enhance the experience, quality of life, and survival of MS patients in City and Hackney. Utilizing specialist clinical skills and evidence-based practice, you will ensure appropriate interventions at each stage of the disease to optimize patient outcomes.
Your responsibilities will include providing specialist advice and support for symptom management, promoting self-management techniques, and effectively networking across health, social services, and voluntary sectors. The goal is to optimize resources, facilitate seamless care pathways, and reduce non-elective hospital admissions and institutional care.
