A local government organization in Northampton is seeking a practitioner to support young people's transition into adulthood. The role involves developing links with schools, conducting assessments, and managing cases. Candidates should have a degree or equivalent experience in adult social care and a passion for empowering young people with additional needs. Training will be provided, and a driver's license is required.

You will be part of the Moving into Adulthood Service and will be aligned with and working in one of the adult social care community teams.

What will you be doing?
You will be instrumental in developing and strengthening our links with SEN schools and FE Colleges to support early identification of young people who may need adult social care support after turning 18. This includes attending EHC reviews and school transition events, and providing advice and presentations to parents and young people from year 9 onwards on topics related to moving into adulthood.
You will also oversee young people who are open to children's social care and health services, primarily from age 16 upwards, with some involvement from earlier ages. You will manage a caseload, conducting timely strengths-based assessments, support planning, and implementing support under the Care Act 2014, ensuring young people and their families are well-prepared for the transition.
Developing strong relationships with partner agencies, including education, children’s, and health colleagues, you will support other adult social care team members by providing advice and guidance. Additionally, you will serve as a key source of management data, informing future planning.

About you
You don’t need experience in all aspects of the role, as training will be provided. However, you should be an experienced practitioner working with young people transitioning into adulthood, capable of demonstrating how you support positive transitions. You should be passionate about working with young people with additional needs and complex conditions, advocating for strengths-based practice, positive risk-taking, and outcome-focused support planning, empowering young people to control their lives and achieve their goals.
You should have a solution-driven, adaptable approach, thrive in a busy environment, and be positive about challenges. Confidence in using your initiative and creativity, analytical skills, and the ability to weigh different perspectives are essential. A driving license and transport are required.
You will hold a degree or equivalent/QCF level 5 with experience in adult social care, or demonstrate knowledge through practice and application of procedures.
This role is exempt from the Rehabilitation of Offenders Act 1974 and requires an enhanced DBS check, as well as a Children and Adults Barred List check. West Northamptonshire Council is committed to safeguarding and expects all staff and volunteers to share this commitment.
