Band 7 | 37.5 hours | 12-month Fixed Term Contract Working across the Frimley Integrated Care System (ICS)

We are delighted to offer an exciting opportunity to join the Neurology Team at Frimley Health NHS Foundation Trust, where innovation and collaboration are at the heart of what we do.

As a specialist member of the Neurology team, you will play a pivotal role in supporting patients living with Motor Neurone Disease (MND), as well as their families and carers, throughout every stage of their care journey. You will work closely with a wide range of professionals and services, ensuring seamless, patient-centred care.

Key aspects of the role include:

  • Managing a caseload of MND patients independently across the Frimley ICS.
  • Conducting home visits and providing holistic assessments.
  • Running outpatient clinics at Wexham Park Hospital and Frimley Park Hospital.
  • Actively participating in MND multidisciplinary team (MDT) clinics with Consultant Neurologists.
  • Attending MDT meetings and coordinating input from a range of professionals.
  • Arranging assessments, onward referrals, and ensuring timely care coordination.

Main duties of the job
  • Take a lead role in the care and service delivery for patients with Motor Neurone Disease (MND)
  • Review and support patients at the point of diagnosis, ensuring timely and sensitive communication
  • Act as the primary point of contact for patients, their families, and carers, providing ongoing advice, support, and guidance
  • Co-ordinate care effectively with the multi-disciplinary team to ensure patients receive holistic and seamless support
  • Support patients in their own homes and during hospital admissions, particularly during periods of symptom progression or deterioration
  • Develop and implement services aimed at reducing avoidable hospital admissions and readmissions, ensuring care is patient-centred and responsive to individual needs
  • Contribute to service improvement initiatives, supporting innovation and best practice in MND care delivery

Qualifications

Essential:

  • NMC or HCPC Registration
  • Relevant specialist post basic programme in an area of neurology or related speciality

Desirable:

  • Completed / completing a Master's degree
  • Teaching / assessment programme
Skills and Knowledge

Essential:

  • Demonstrates up to date knowledge and expertise in general nursing/physiotherapy/ Occupational therapy and speech and language
  • Demonstrates understanding of relevant national strategy / policy and how this relates to the service
  • Knowledge of key professional issues and NMC/HCPC guidelines and codes, relating to professional practice.
  • Care of patients with neurological disorders, respiratory failure, nutritional requirements, swallow difficulties, progressive neurological term conditions

Desirable:

  • Non-invasive ventilation
Experience

Essential:

  • Evidence of previous relevant clinical experience (postgraduate) in the same or related specialty
  • Evidence of recent clinical experience in related speciality
  • Experience managing neurological disorder patients in community or hospital settings

Desirable:

  • Experience of applying research evidence to clinical practice
  • Previous community experience
Special Requirements

Essential:

  • Full driving licence and access to a car to travel across sites and perform domiciliary visits
