A community health organization in Leicester is seeking a committed Nurse Specialist to provide maternity cover within their Palliative Care Team focused on Motor Neurone Disease. The role involves home visits, specialist patient care, and collaboration with multi-disciplinary teams. Candidates must demonstrate strong interpersonal skills and a commitment to high-quality care, along with relevant nursing qualifications and experience in a community setting. We are seeking a committed and compassionate Nurse Specialist to provide maternity cover within our Motor Neurone Disease (MND) Palliative Care Team. You will need to have well developed interpersonal skills, experience and be passionate for caring for patients with MND and/or other neurological conditions, in community settings, or within Palliative and end of life care needs.
Due tothe nature of the role and visiting patients in remote areas it is essentialthat you hold a current driving licence and have access to a car/transfer forwork purposes.
Current NHS pension scheme members can continue in the scheme under LOROS\' direction body status and NHS service will be taken into consideration in relation to annual leave entitlement.
We are looking for a nurse who will:
Act as a key worker, experienced in caring for patients with Motor Neurone Disease, supporting individuals and their families from diagnosis through to end of life. Experienced in caring for patients.
Passionate about delivering high-quality, person-centred care.
A confident communicator with strong teamwork and independent working skills.
Work collaboratively with the multi-disciplinary team at LOROS and across the wider health and social care community.
Deliver specialist care across Leicester, Leicestershire and Rutland.
Contribute to training programmes for healthcare professionals and carers.
Take part in opportunities for personal development, service improvement and clinical audit.
LOROS Hospiceis a local charity and every year we care for over 2,500 people acrossLeicester, Leicestershire and Rutland. We deliver free, high-quality,compassionate care and support to terminally ill patients, their family andcarers.
Clinical
1.Undertake home visits and complete holistic specialist needs assessments.
2.Provide specialist intervention to supportpatients and carers.
3.Act as a resource for the primary health careteam, other specialist teams statutory organisations and voluntaryorganisations on issues relating to the care of patients with MND.
4.Provide a telephone advice service to patients,carers and professionals.
5.In collaboration with the multi-professionalteam, plan the provision of care based on a holistic needs assessment.
6.Manage nurse led MDT meetings and undertaken nurse led MND clinics.
7.Liaise with other staff and departments at theHospice, and refer to otheragencies that can provide additional forms of support for example, MNDA.
8.In conjunction with other members of theMulti-disciplinary Team (MDT), monitorthe progress of all MND patients.
9.Provide information to patients and carers toenable them to make informed choicesregarding care.
10.Comply with the NMC code of practice and adhereto all policies and procedures of theLeicestershire & Rutland Hospice.
11.Comply with Health and Social Care Act 2008/CareQuality Commission.
12.According to LOROS Hospice Policy and where theNurse Specialist has undergone training,prescribe medication for patients.
Education
1.Participate in any relevant training programmesfor any professionals or non-professionals both within Leicester,Leicestershire & Rutland and nationally and as required.
2.Participate in in-service education programmesand be responsive to external influences and new initiatives.
3.Participate in education programmes forprofessional visitors on placement.
4.To participate in the provision for information.
5.Attends mandatory training and is responsiblefor developing own personal development plan.
6.Provide co-support to team colleagues.
7 Participatein clinical supervision and reflective practice sessions.
Research & Audit
1.Participate in any multi-professional audit,which evaluates the effectiveness of the care, provided by LOROS NurseSpecialist Team.
2.Participate in and initiate multi-professionalaudit.
3.Be aware of new developments in the care ofpatients with MND and implement appropriate research based practice.
4.Keep up to date with current advances in nursingpractice and research and to take responsibility for his/her own professionaldevelopment (PREP) and maintaining registration.
5.Participate in the development of standards forthe MND Nurse Specialists and for the wider CNS team.
Managerial
1.Be an active member of the LOROS Community NurseSpecialist Team, contributing to team initiatives and developments.
2.Contribute towards development of strategies forMND services within Leicester, Leicestershire & Rutland.
3.Participate in appropriate Multi-professionalmeetings.
4.Demonstrate effective day to day planningthrough efficient management of self, caseload and resources.
5.Maintain accurate standardised electronicpatient records for activity, evaluation, research and audit purposes.
6.Maintain confidentiality and conform to the DataProtection Act.
7.Comply with requirements of the Health andsafety at Work Act and comply with risk assessments monitoring andmaintaining health, safety and security of self and others.
