Job description

The Peninsula MND Care and Research Network is seeking a new care co-ordinator to join their team.

We are recruiting a Band 7 professional to work within the MND team covering parts of Devon, depending on their home location. The role involves managing a caseload of MND patients independently, visiting patients at home, running MND MDT clinics, participating in MDT meetings, and coordinating assessments and referrals. The position offers opportunities to develop clinical management skills and potentially take on more managerial responsibilities in the future, helping to shape MND care in the South West.

The role includes involvement in clinical research, education, and audit activities. The successful candidate will support patients with complex emotional and physical needs according to MND Gold Standard guidelines, facilitate MDT models, and serve as the Devon-wide clinical specialist providing support and advice to care teams.

Additional responsibilities include developing education programs for MDT teams across various settings, providing information and support to patients and carers, and supporting patients through their journey from diagnosis to end of life care. The role also involves working with the MND Association to support research initiatives and develop clinical research for MND patients.

The post offers a pathway to gain the necessary skills for clinical management of the service, working closely with the current lead.
