Job description

The Peninsula MND Care and Research Network is seeking a new care co-ordinator to join their team.

We are recruiting a Band 7 professional to work within the MND team covering parts of Devon, depending on the candidate's home location. The role involves managing a caseload of MND patients independently, with opportunities to develop skills and eventually take on some clinical management of the service once confident with caseload work. Responsibilities include visiting patients at home, running MND MDT clinics, participating in MDT meetings, arranging assessments and referrals, and coordinating care for individuals with Motor Neurone Disease. The post is partly funded by the MND Association.

This position offers an excellent opportunity for someone with strong clinical, communication, and presentation skills, who is interested in a role with managerial elements in the future and aims to influence MND care in the South West region. Innovation, enthusiasm, and passion are essential qualities.

The role also involves clinical research, education, and audit activities.

  1. Support a large caseload of patients with complex emotional and physical needs following specialist guidelines for MND Gold Standard care.
  2. Facilitate the MDT model through clinics and team meetings.
  3. Act as the Devon-wide clinical specialist for MND, providing support and advice to care teams.
  4. Develop and deliver education to MDT teams across community, hospital, hospice, and care home settings locally and nationally.
  5. Provide information, support, signposting, and education to patients and carers.
  6. Support patients and carers throughout their journey from diagnosis to end of life care.
  7. Gain knowledge of the MND Association and its support roles.
  8. Collaborate with the MND Peninsula Research Nurse to develop clinical research and maximize patient involvement.
  9. Acquire skills to eventually take on clinical management responsibilities alongside the MND Service Coordinator, sharing strategic leadership, and learning necessary clinical skills and knowledge.
