Job description

Moorlands and Rural PCN Salaried General Practitioner

Main tasks/overview of responsibilities: In accordance with the PCN rota, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing/authorising repeat prescriptions, and dealing with queries, paperwork, and correspondence in a timely manner. The post-holder will make professional, autonomous decisions regarding presenting problems, whether self-referred or referred from other healthcare workers within the organisation. They will assess the healthcare needs of patients with undifferentiated and undiagnosed problems, screen patients for disease risk factors and early signs of illness, and develop care plans for health in consultation with patients, following current practice disease management protocols. Providing counselling and health education, admitting or discharging patients to and from the caseload, and referring to other care providers as appropriate are also key responsibilities. The post-holder will record clear and contemporaneous consultation notes to agreed standards, collect data for audit purposes, and compile and issue computer-generated acute and repeat prescriptions in accordance with the practice prescribing formulary or generically when clinically appropriate. The role may require visiting patients during surgery time in emergencies and participating in on-call sessions as per the practice rota, including PCN Enhanced Service hours (Monday to Friday 6:30pm to 8pm and Saturday 9am to 5pm). They will attend training and meetings organised by the practice or other agencies and support medical students and other training roles. Managing patient care involves creating opportunities to address needs expediently, balancing continuity of care with service access, and encouraging the use of telephone and other technologies to optimize face-to-face consultations. Patients should be reassured about care options and safety concerns should be promptly communicated to the Clinical Director, Practice Partners, or PCN Manager.
