Job summary

Shropshire Community Health NHS Trust in Shrewsbury South locality is seeking a motivated individual to join the Community Team in planned admissions avoidance care, delivering phlebotomy and general support in patients' homes under supervision of a nurse.

You will work in a busy team, requiring commitment to interdisciplinary collaboration, strong communication and ability to work autonomously while maintaining high standards of care.

Qualifications

• Educated to GCSE level or equivalent.
• Experience of working in a health care setting and/or delivering phlebotomy.
• Ability to communicate effectively, both verbally and in writing.
• Understanding of clinical governance, risk management and autonomy.

Responsibilities

• Promotes a can-do attitude to develop and sustain a dynamic community team.
• Organise and deliver skilled clinical support work / phlebotomy without direct supervision.
• Gain valid informed consent and work within legal frameworks with patients.
• Liaise with nursing and therapists regarding patient care and highlight variations to expected outcomes.
• Document patient interventions/findings in patient records per Trust policy.
