Job summary

Qualifications

• Essential: current professional registration with appropriate body in healthcare or social work.
• Post-graduate qualification and clinical supervision experience.
• Willingness to study towards NMP and leadership qualifications.

Responsibilities

• Provide high quality assessments, interventions and care management across settings.
• Integrate into different PCN teams to ensure continuity of care.
• Lead and collaborate with multi-disciplinary teams across networks.

Education

HCPC / NMC / Social Work England registration

Post-graduate qualification in relevant area

Clinical Supervisor qualification

Mentoring / teaching / assessing in clinical practice

ITQ level 2 or equivalent

Excellent literacy and numeracy
