A regional NHS healthcare provider is seeking a Transition Support Worker to facilitate the transition of 17-18 year-olds from Children's mental health services. The role involves developing care plans, advocating for young people, and collaborating with professionals. Ideal candidates will have excellent communication skills and a deep understanding of mental health services. This position offers flexibility and aims to enrich diversity within the workforce.
This role will be part of the Transition Pathway within the CYPS Mindworks Community Hub. The Transition Pathway provides support for 17- and 18-year-olds as they transition out of Children's EWMH Services. The transition pathway is for those who need additional support but the appropriate service to meet their needs is unclear. The Transition Support Worker will case manage and co-ordinate support for young people aged 17-18 as they transition, acting as a reliable and consistent advocate over a period of 6 months. They will develop a Transition Care Plan with the young person to identify needs and ensure the right help and support is provided during the transition period and beyond. They will accompany the young person to appointments, assessments and therapy, educate and reflect on differences in culture and practice between services, and help signpost and/or refer to emotionally beneficial services. The successful candidate will attend regular MDT meetings and liaise with a range of professionals, agencies and other parties involved in the young person’s care.
