Job summary

We are looking for dedicated, highly motivated, skilled and enthusiastic Community Senior Practitioner to join our busy Intensive Home Treatment Team in County Durham and Darlington. We provide assessment and treatment for older persons with functional and organic illness within the community setting, including supporting patients after discharge from acute hospitals, mental health hospitals, and intermediate care beds. This role involves supporting patients and carers in their own homes and providing telephone support during crises.

You will need excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.

There is an expectation of commitment to high-quality care development and engagement in quality improvement activities. You will receive regular management and clinical supervision and have access to training and development opportunities.

Due to the role's nature, applicants must be able to travel independently across the locality and have access to a suitable vehicle for work purposes. Adjustments can be considered under the Equality Act 2010.

Candidates must have at least 18 months post-qualifying experience, including sign-off of relevant preceptorship or competency frameworks.

Main duties of the job

  • Promote and maintain safety, privacy, and dignity of all patients, respecting differences including spiritual and cultural beliefs.
  • Obtain patient consent for care and treatment.
  • Recognize signs of patient harm, abuse, or neglect.
  • Maintain high professional standards and attitudes towards patient care.
  • Assess patients with complex needs and develop, implement, and evaluate intervention plans.
  • Base intervention plans on current risk assessments and evidence-based practice.
  • Undertake risk assessments and devise appropriate actions and plans.
  • Support patients within community settings, liaising with support services.
  • Provide health education and promotion advice to patients and carers.
  • Make decisions and coordinate in crisis situations, seeking advice from relevant professionals when needed.
