Our Constitution Street service delivers high-quality Care at Home and Housing Support services for adults with severe and enduring mental health challenges. Supporting 11 individual tenancies and providing community-based outreach support, the service operates on a 24/7 basis.
We aim to equip people with skills needed to live independently and meaningfully within the community. We use a strengths-based coaching approach to recovery, building confidence, maintaining motivation, and promoting resilience. We also provide practical support with food preparation, medication, budgeting, and daily living skills. Promoting participation and inclusion is a core value, and we work alongside people to deliver innovative, creative, and personalized interventions and opportunities.
You will be experienced and passionate about working in a person-centred way with adults experiencing mental health issues or expressing needs through distressed behaviour and substance misuse. With a strong interest in people and a commitment to helping others, you will strive to positively impact lives and tailor support to enable individuals to reach their potential.
The service operates 24 hours, requiring a variety of shifts on a rota basis, including paid sleepovers.
