Job summary

Qualifications

• Post-qualification experience in adult mental health services.
• Experience completing biopsychosocial assessments and risk formulations.
• Ability to make autonomous clinical decisions while managing priorities.

Responsibilities

• Triage routine adult mental health referrals from SPA with biopsychosocial assessment.
• Assess risk, formulate safety needs and escalate as required.
• Apply clinical pathways and seek senior support for complex decisions.
• Present triaged referrals at daily meetings and document outcomes.
• Communicate decisions clearly to patients, carers and referrers.
• Refer onward to Hub pathways, primary care and local services as needed.
• Manage a caseload or act as keyworker when required and ensure safe discharge.
• Provide supervision to junior colleagues and students where appropriate.
• Deliver psychosocial interventions and promote recovery-focused practice.
