About us

We bridge the gap between in-patient mental health services and independent living. Bridge Support offers cost-effective, proven pathways to support people with long-term mental health conditions in the community. We are looking for a new team to support our latest service in Kingston Upon Thames.

Job Purpose

This role involves supporting clients transitioning from hospital to their homes, offering flexible assistance for individuals with enduring mental health needs on a 7-day shift rota. The focus is on guiding clients through their recovery journey over a 12-month period, helping them set and achieve their goals, become more self-reliant, and reduce their dependence on medical interventions and hospitalisations. The role also includes providing outcome-focused support, addressing dual diagnosis issues, managing housing-related aspects, and collaborating across various services and sites as needed.

Key Responsibilities include:

  • Engage with clients within established boundaries in order to build rapport
  • Be responsive to the presentation of clients and take reasonable precautions to manage risk, including deescalating and seeking appropriate assistance, when necessary from emergency services
  • Maintain links with appropriate support services for clients and staff, e.g. GPs, Social Services, Registered Social Landlords, or other relevant housing association or landlords, Community Mental Health Teams, Substance Misuse Services, Commissioners (where appropriate), leisure, educational or work activities
  • Assisting with health and safety checks, log and reporting property repairs and maintenance
  • Ensure all written records with up-to-date, clear, comprehensive and appropriate information
  • 22 days holiday plus bank holidays (increasing to 25 days after 3 years’ service)
