Job summary

Midlands Partnership University NHS Foundation Trust is seeking a Level 1 Band 5 Social Worker to contribute to the Adults' Social Care Pathway. The post is based primarily in Stafford with occasional work in Cannock and other South Staffordshire areas as required.

The role involves assessments of need, collaboration with carers and multidisciplinary teams, and supporting professional development through supervision and the SWLA/PCF framework. Registration with Social Work England is required.

Qualifications

• Registered Social Worker with Social Work England
• Commitment to ASYE programme
• Experience with adults/older adults with mental health needs preferred
• Good keyboard/IT skills (Word, Excel, PowerPoint)

Responsibilities

• Carry out assessments of need and explore how best to meet these needs
• Work with carers, families, other agencies, the local community and MDTs to ensure continuity and high-quality service
• Engage in supervision and appraisal to support professional development and SWLA/PCF alignment
• Be flexible to work across Staffordshire areas (Stafford, Cannock, Codsall) as demand requires
