Adult Community Mental Health Team

The Adult Community Mental Health Team based in Bury St Edmunds is excited to welcome applications for a year long secondment from Senior Mental Health Practitioners (Nursing/OT/Social Work) with experience or keen interest in working with complex mental health conditions. Endless opportunities are available and we are keen to welcome new faces to join our team on a permanent basis.

Responsibilities
  • Managing a complex caseload for those under The Care Programme Approach
  • Undertaking initial urgent/routine assessments on behalf of the service
  • Attending multi‑disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant Psychologists
  • Attending outpatient clinic appointments
  • Undertaking face to face visits in the community setting including home visits
  • Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
  • Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management
  • Working with needs types and many evidence based work books to enhance the Service user experience
  • Involved in the clinical leadership decisions as part of the senior leadership team to make sure that KPIs are met and team goals are being worked towards
  • Supervision and mentorship of junior staff members
