Overview

A Vacancy at Greater Manchester Mental Health NHS Foundation Trust. This is a fantastic opportunity to be part of the Wigan referral and assessment team. We are looking for skilled practitioners, with mental health experience, who are able to complete triage and assessments at the front door service into secondary care mental health.

Successful candidates will play an important part in the delivery of high-quality service provision. As part of the team, you will be empowered to shape and build the service as community transformation develops in Wigan. You will help build and maintain positive working relationships with colleagues and the wider network.

Responsibilities
  • Provide clinical provision of rapid assessment and care/pathway planning for people referred into the R&A presenting with mental health problems.
  • Carry out mental health assessments, accessing all available sources of information and including, where appropriate, the views of family and carers when relevant.
  • Undertake and record comprehensive needs/risk assessments, developing clear goals and timeframes for onward interventions with the service users.
