Main duties of the job

As a mental health practitioner in one of our teams you can expect to:
- Participate as a member of a multi-disciplinary team managing a caseload of patients
- Undertake assessments of patients, involving theircarersand family members in the patient's care and care plans.
- Be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
- Supervise Support Workers and/or students.
- Provide advice to other members of the service and agencies on the needs of service users.
