Job description
Overview

Working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need. Delivering a high quality clinical service to patients and their families. Care coordination of an identified caseload with access to regular and robust supervision. Delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

The Integrated Mental Health teams work as part of a 'neighbourhood' offering place based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.

Responsibilities
  • Assess care needs and contribute to the formulation of the person’s needs and development of a relevant plan based on negotiated goals and evidence-based interventions.
  • Work with the multi-disciplinary team to provide nursing interventions to support the person to meet their agreed goals and coordinate and/or participate in reviews of progress, using recommended outcome measures and involving the service user and any identified carer throughout the clinical pathway.
  • Coordinate and/or participate in reviews of progress towards goals and ensure involvement of service users and carers as appropriate.
  • Travel across a large geographical area as required by role; post holder will work Monday to Friday 9am – 5pm and travel will be across the area.
Qualifications & Requirements
  • Mentally Health Nurse (RMN) or Occupational Therapist; experience with community mental health settings preferred.
