A Vacancy at Greater Manchester Mental Health NHS Foundation Trust.


An opportunity has arisen for Band 6 Registered Mental Health Nurses, Social Workers or Occupational Therapists to join the Salford Home Based Treatment Team. This dynamic and driven multi-disciplinary team offer an alternative to psychiatric hospital admission, supporting service users in acute mental health crisis in their homes, as well as supporting loved ones and carers during this period. The team are passionate about providing person centred care, tailoring care plans to suit the needs of all individuals involved during the care spell and providing a range of psychosocial interventions during their admission. The team are required to thoroughly risk assess situations and have access to a supportive leadership team to support such complex cases.

Staff wellbeing and development is paramount and well supported in this team. Full supernumery induction is provided for the first 4 weeks in post. The post holder will be supported with their professional development as part of their appraisal process, and regular 1:1 supervision is provided. The post holder may also like to become actively involved in a number of service development projects as part of their role.

The team operates a 24/7 service, therefore there will be a requirement to cover shift work including nights. Flexible working requests can and will be supported however possible.

Main duties of the job

- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.

- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.

- To act as keyworker for a small caseload of service users, with support from the multi-disciplinary team.

- Cover the duty / shift coordination role on a regular basis.

- Liaison with other agencies involved in a service users care.

- Lead on the discharge or transfer of care for our service users to appropriate agencies.

- To support and supervise junior members of staff.
