Mental Health Practitioner - Home Based Treatment Team

An opportunity has arisen for a Band 6 Registered Mental Health/Learning Disability Nurse, Social Worker, or Occupational Therapist to join the North Home-Based Treatment Team to cover maternity leave. This dynamic, multi-disciplinary team offers an alternative to psychiatric hospital admission, supporting service users in acute mental health crises in their homes, as well as supporting loved ones and carers during this period.

The team is passionate about providing person-centred care, tailoring care plans to meet individual needs, and delivering a range of psychosocial interventions. The team thoroughly risk assesses situations and is supported by a leadership team to manage complex cases. Staff wellbeing and development are prioritized, with full supernumerary induction provided for the first 4 weeks. The post holder will be supported in their professional development through regular supervision and appraisal, and may participate in service development projects.

The team operates a 24/7 service, requiring shift work coverage.

Main duties of the job
  1. Collaborate with service users and their carers in care planning.
  2. Conduct risk assessments and record risks according to trust policy.
  3. Participate in shift coordination when allocated.
  4. Engage in multidisciplinary team meetings and agree care plans with patients.
  5. Maintain knowledge of evidence-based home treatment interventions and deliver them accordingly.
  6. Ensure service users are treated in the least restrictive environment through gatekeeping assessments.
  7. Assist in shift allocation and coordination.

Job responsibilities

Shift patterns include:

  • Saturday (midnight to midnight) and any weekday after 8pm and before 6am
  • All hours on Sundays and Public Holidays (midnight to midnight)
