A leading healthcare provider in Manchester seeks a Mental Health Practitioner for its Home Based Treatment Team. The role supports individuals in acute crises at home, emphasizing personalized care and development opportunities. The practitioner will coordinate shifts and work in a dynamic team centered on patient wellbeing.
An opportunity has arisen for a Band 6 Registered Mental Health/Learning Disability Nurse, Social Worker, or Occupational Therapist to join the North Home-Based Treatment Team to cover maternity leave. This dynamic, multi-disciplinary team offers an alternative to psychiatric hospital admission, supporting service users in acute mental health crises in their homes, as well as supporting loved ones and carers during this period.
The team is passionate about providing person-centred care, tailoring care plans to meet individual needs, and delivering a range of psychosocial interventions. The team thoroughly risk assesses situations and is supported by a leadership team to manage complex cases. Staff wellbeing and development are prioritized, with full supernumerary induction provided for the first 4 weeks. The post holder will be supported in their professional development through regular supervision and appraisal, and may participate in service development projects.
The team operates a 24/7 service, requiring shift work coverage.

Salary: £37,338 to £44,962 per annum, pro-rata.
